The post holder will be substantively employed by Nottingham CityCare Partnership CIC within the Community Beds Team. However this role forms part of an Integrated Care System (ICS) rotational programme and the post holder will be expected to undertake rotational placements across partner within ICS health and social care organisations. Dimensions Responsible for Occupational Therapy assessment and treatment for a defined clinical caseload appropriate to the service area. Working in conjunction with and under supervision of a senior OT and other senior clinical professionals. Lead group sessions where appropriate. Able to fulfil physical demands of role, including manual handling applications. Will be required to travel city-wide for client visits, meetings and training. Main Duties and Responsibilities Medicines reconciliation post discharge and on other transfers of care, ensuring recommendations are within local guidelines/formularies and not conflicting with existing medication Undertake individual assessments of patients and identify their medicines management needs. Ensure effective liaison with colleagues across health and social care to establish a suitable level of assistance Undertake medication use reviews, identifying and finding solutions for non-compliance Demonstrate a sound understanding of the needs of frail, older and vulnerable people, including those with disabilities and promote their independence with regards to medicines management, advising on aids to independence as appropriate.
