Summary
Job Purpose To contribute to a high quality service by carrying out assessment, goal setting, implementing treatment programmes and evaluation of adult clients who may have complex and/ or chronic presentations including musculoskeletal, sensory impairment, medical and mental health needs, with support and supervision from senior physiotherapists. To work within Community Beds Team withing Health Reablement service . This may extend or change to other community services in-line with organisational development To support clients to improve or maintain their level of function and quality of life and plan future care in an appropriate setting in partnership with multi-disciplinary team (MDT) colleagues, including liaising, supporting and referring on to health services, voluntary services and public sector agencies. To participate actively in the services training strategy, including in-service training, organisation-specific and multi-agency training. To be responsible for planning a safe discharge from the service. To act as Key Worker as required. The post holder will be substantively employed by Nottingham CityCare Partnership CIC within the Community Beds Team. However this role forms part of an Integrated Care System (ICS) rotational programme and the post holder will be expected to undertake rotational placements across partner within ICS health and social care organisations. Dimensions Responsible for Occupational Therapy assessment and treatment for a defined clinical caseload appropriate to the service area. Working in conjunction with and under supervision of a senior OT and other senior clinical professionals. Lead group sessions where appropriate. Able to fulfil physical demands of role, including manual handling applications. Will be required to travel city-wide for client visits, meetings and training. To participate in rehabilitation treatment programmes with the MDT To carry out training of other members of staff. Support senior colleagues with the facilitation of student placements. WLD specific: To undertake case management responsibilities that include assessments of need, within a Whole Life, strengths based person centred approach, monitoring, coordinating and reviewing of care and support plans being able to reach down in working with children, young people and adults as part of preparation for adulthood.
