This is an exciting opportunity for the right person to work with a range of professionals and people to support the principles of Home First. The worker must have the ability to work as part of a team in a pro-active manner identifying opportunities for timely discharge whilst ensuring the patient has the appropriate level of support, which may incorporate the use of technology upon discharge from hospital. Tasks will be carried out in partnership with service users, family members, colleagues within Social Policy and other professionals and agencies as necessary to meet need. The worker will be required to have a good understanding of Self-Directed Support and the importance of completing outcome focused assessments. The worker will be required to undertake essential training including in adult support and protection training. The worker must have the ability to travel throughout West Lothian.
