We have an exciting opportunity for an independent, autonomous Prescribing Clinician to play a key role in our Care Coordination and Integrated Urgent Care services across the Bath, Swindon and Wiltshire locality on a permanent basis. Our Care Coordination shifts are based at our Chippenham Head Office and our out of hours sessions will be based at one of our Primary Care Centres.
