An experienced and exceptional Contract Lead who can lead on contract management of our Hospital Discharge contracts, working with our NHS partners and providers to ensure there is a range of services available for people coming out of hospital, reviewing arrangements and supporting ongoing commissioning activity. The teams work closely with partners and providers to ensure services are in place to support people to be discharged from hospital when they are ready and no longer need hospital care. Commissioning ensures that there are a range of services to support people, and where possible people are supported to return home with the care and support that they need. About you You will have a strong understanding of the important role that Adult Social Care can play in people's lives and a commitment to making the customer experience better by working collaboratively with partners and providers. You will have strong analytical, literacy, problem solving and project management skills to manage contracts and produce technical reports which make evidence-based recommendations. You will have strong interpersonal, influencing and persuasion skills to work across organisations and the market to deliver improvements and change. You will have experience or an excellent understanding of working in a public sector environment. You will have a degree or equivalent qualification or experience which demonstrates that level of applied knowledge and understanding to deliver the role. We are seeking dedicated individuals to provide overnight support in a calm, home environment. This is a rewarding opportunity to make a real difference in someone's life. In this role, you will support a client who relies on a ventilator, ensuring their safety, comfort, and well being throughout the night. You'll closely monitor their condition, remain alert, and respond promptly and professionally to any changes.
