Summary

37 hours per week, Fixed Term to 31 March 2029.

Monday to Friday, including evenings and some occasional weekend working

Are you passionate about helping people achieve a healthier weight and make lasting changes to their health?

Active Sunderland is looking for a Weight Management Support Officer to deliver a community-based healthy weight programme that supports adults to reach a healthy weight, become more active and improve their overall health and wellbeing.

Through one-to-one appointments, practical advice and ongoing encouragement, youand#39;ll help people develop healthier habits, overcome barriers and make sustainable changes.

In this role, you will:

Deliver one-to-one appointments over a 26-week programme.

Support adults to achieve and maintain a healthier weight.

Provide advice and guidance on healthy eating, physical activity, and wellbeing.

Deliver weekly health education and low-intensity physical activity sessions.

Build relationships with health and community partners to promote the programme and encourage referrals.

Record and monitor outcomes to demonstrate the impact of the programm

Weand#39;re looking for someone who is enthusiastic, approachable and experienced in supporting behaviour change. andnbsp;Youand#39;ll have experience of delivering one-to-one support, and motivating people to make positive, sustainable changes to improve their health.
