We are seeking a dedicated and experienced CSW - Senior Social Worker who is socially care qualified to join our dynamic team. As a Senior Social Worker, you will take a leadership position within our social care department, providing guidance, assessment, and intervention to support vulnerable clients. You will be responsible for managing complex cases, ensuring safeguarding practices are upheld, and delivering personalized care plans tailored to individual needs. Responsibilities
Conduct comprehensive assessments of individuals and families to determine needs and risks.
Develop and implement personalized care plans in collaboration with clients and multidisciplinary teams.
Provide leadership and mentorship to junior social care staff and colleagues.
Ensure that all safeguarding protocols and legal requirements are strictly followed and documented.
Advocate for clients to access appropriate resources and services to improve their well-being.
Maintain clear, accurate, and timely records and case notes in compliance with organizational standards.
Collaborate with external agencies, healthcare professionals, and community organizations to coordinate care.
Participate in continuous professional development and contribute to service improvement initiatives.
