Ensure that residents Care Plans are completed and maintained in conjunction with Clients, relatives and other Health Care Professionals, and in accordance with NMC guidelines & Company policy. Assess all aspects of residents’ care needs, and provide health supervision and direct Nursing are when required. Assess social and health care needs of new residents, and maintain on-going assessment and review of all other residents. Maintain accurate records and ensure that each resident within the Care Home has an individualised Care Plan. Resident Care Plans to be reviewed on a monthly basis, or more frequently as and when required, in accordance with NMC guidelines & Company policy. Carry out regular checks on Residents at intervals determined by the Home Manager. Ensure a clear and concise handover report is given to all relevant Staff Members at the end of each shift. Administer/Order/Store prescribed medicines and document the same in accordance with the Company’s procedure and NMC guidelines. Practice maximum integrity in all dealings with resident’s personal and financial affairs, and avoid abuse of the privileged relationship which exists with residents. Establish and maintain good communication with residents, relatives and with the multidisciplinary team. Provide administrative support when required. Adhere to all appropriate NMC guidelines / regulations. Maintain all logbooks and records as required by both the Registering Authority and the Directors of the Company. The post holder will provide comprehensive specialist assessments, including risk assessment of young people and their families/carers referred with mild learning disability and/or ASD and/or ADHD. They will provide evidence-based behaviour management interventions as part of an integrated care package to children, young people and their families/carers. The post holder will safely prescribe and monitor medications within local and national guidelines, in liaison with a Consultant Child & Adolescent Psychiatrist. This includes conveying to parents, carers and young people, the implications of a prescribed medication, the side effects and what they have to watch for in a sensitive, meaningful way. You will be expected to demonstrate a working knowledge of the prescription of medication effects, side effects and contra-indications in accordance with the independent and supplementary prescribers’ training and current legislation. They will be aware of key trends in the evidence base for prescribing and maintain an up-to-date knowledge of medicines commonly prescribed. We have an exciting opportunity within our Family Hubs and Children's centres for a Speech and Language Therapist with specialist skills and knowledge in Early Years service delivery for children aged 0-5 years of age. The post is an integral part of the delivery of early idnetification, early support adn early intervention for children with a range of speech , language and communication needs. Work in the Family hubs is aimed at health promotion within the SLCN pathway. The Early Years’ team offers specialist assessment, intervention and advice for younger children. This includes children with social communication difficulties, developmental language disorder, dysfluency and language delay. The post holder will build upon the existing Family hubs offer. They will be involved in providing training and workshops for parents and Family hub staff and implementing evidence based early language programmes. To work closely with more senior colleagues to develop and run a service to families in Family Hubs and Children Centres promoting Early Communication. To independently use specialist skills and knowledge, to work jointly with family hub staff and more senior colleagues to develop services to support the identification and early intervention for the development of speech language and communication skills in children 0-5 years of age. To independently manage a large and diverse caseload in the Family hubs, providing high quality assessment and intervention, reporting directly to the Clinical Lead when required. To be part of the speech and language service to children of Newham in the area of Early Years. To provide a service which takes account of the needs of clients from a range of ethnic and linguistic backgrounds. To co-ordinate and take a lead in the joint planning and delivery of SLCN interventions including parent workshops, drop-ins, groups and staff training within the family hubs and early years.
