Care Coordination is the joining up of services, coordination, information and communication between caregivers, treatment providers, those living with and beyond cancer and their families, that creates a seamless experience of care (NHS Improvements, 2011). The post holder will support both the Clinical Nurse Specialist (CNS) Team and the Macmillan Information & Support Centre Team in providing exemplary personalised cancer care. The post holder will be required to coordinate with appropriate services to maximise the delivery of the cancer pathways, thereby improving the quality and experience for patients and their carers.
