Summary

An exciting opportunity has arisen for aBand 3 Community Support Workerto join ourBrent Urgent Community Response (UCR) Teamat Central London Community Healthcare NHS Trust.

This is afull-time role, working 37.5 hours per week, supporting the delivery of high-quality care to patients within their own homes and community settings.

Working as part of a supportive multidisciplinary team, you will help provide timely care and support to patients who may be experiencing a change in their health or circumstances. You will play an important role in helping patients maintain their independence, supporting their recovery andpreventing avoidable hospital admissions wherever possible.

We are looking for someone who iscompassionate, reliable and committed to delivering person-centred care. You will enjoy working with people, be able to communicate effectively with patients, families and colleagues, and be comfortable working within a busy community service.

As aBand 3 Community Support Workerwithin the Brent Urgent Community Response Team, you will:

• Provide safe, compassionate and personalised care to patients in their own homes, working alongside registered nurses, therapists and other members of the multidisciplinary team.

• Support patients with a range of nursing and therapy needs, helping them to maintain their independence and remain safely at home wherever possible.

• Carry out delegated clinical activities and interventions under the direction of a registered healthcare professional and within your agreed level of competence.

• Provide holistic, person-centred support, recognising changes in a patient’s needs or condition and reporting concerns appropriately.

• Support the team with relevant clinical and administrative duties to ensure patients receive timely and coordinated care.

• Communicate effectively and sensitively with patients, families, carers and healthcare professionals.

• Maintain accurate patient records and confidently use the relevant IT systems required for the role.

• Work collaboratively as part of the wider community team to deliver a responsive and effective service.

Please note:Afull UK driving licence and access to a car for work purposes are essentialfor this community-based role.
