Experience of working with SEN learners in a secondary or FE environment is desirable. Job description Salary: £26,756 per annum Hours: 37.5 hours Job Type: Permanent – Full-Time Service: Mental Health Location: Lambeth Shift: Monday to Friday - 9am to 5pm About the role We are looking to recruit Reablement Support Workers to provide specialist expertise and support to Living Well Centre teams. They will facilitate the achievement of person-centred outcomes for people living in the community, some of whom have complex mental health and other support needs. Support workers will work collaboratively with professionals, including care coordinators, social workers, occupational therapists, psychologists and psychiatrists, as detailed in their support plans to achieve agreed outcomes and provide best practice support for clients. The support worker will act as ‘social prescribers’, connecting people to local groups and services including peer support. They will also help people reconnect with family and friends.
