Summary

Accountabilities Service Coordination and Operational Support Support the day-to-day coordination of the CCT service under the direction of the Clinical Lead. Act as a first point of contact for patients, carers and professionals, ensuring enquiries are handled efficiently and support is provided where needed. Assist with scheduling, appointments and service activities. Team Liaison and Coordination Work closely with colleagues, GP Practices, PCNs and other healthcare professionals to support coordinated, person-centred care. Share information appropriately, follow up actions and attend team meetings to help ensure patients receive timely support. Service Administration Provide administrative support to the CCT service, including managing correspondence, preparing documents and reports, maintaining filing systems, and supporting governance, performance and operational requirements. Clinical System Administration To develop expertise in the administrative functions of Clinical systems, specifically Systmone and EMIS, including problem solving, staff support, access, reporting and contingency. Record Management and Compliance Maintain accurate and up-to-date patient and service records in line with information governance, GDPR and confidentiality requirements. Support audits, monitoring activities, complaints and quality assurance processes to help ensure the service remains safe, effective and compliant.
