Summary

Clinical Duties Conduct face-to-face, telephone, and home visit consultations in line with practice requirements. Assess, diagnose, and manage patients presenting with a wide range of health conditions. Make autonomous clinical decisions and develop appropriate treatment and care plans. Screen for disease risk factors and promote health education and preventative care. Prescribe safely and in accordance with local and national prescribing guidelines. Maintain accurate, timely, and contemporaneous clinical records. Manage referrals, admissions, discharges, investigations, and follow-up care as appropriate. Support clinical audit, quality improvement, and evidence-based practice. Professional & Organisational Responsibilities Comply with all practice policies, including confidentiality, safeguarding, information governance, health and safety, and infection prevention and control. Work collaboratively within the multidisciplinary team to deliver high-quality patient care. Participate in practice meetings, training, appraisal, and continuing professional development. Contribute to service development, clinical governance, and quality assurance activities. Support effective use and maintenance of electronic patient records. Communication & Teamwork Communicate effectively and professionally with patients, carers, colleagues, and external agencies. Promote equality, diversity, dignity, and respect in all interactions. Respond appropriately to patients' communication needs and ensure inclusive access to services. Quality, Safety & Governance Maintain high standards of clinical care, patient safety, and infection control. Identify and report risks, incidents, and safeguarding concerns in accordance with practice procedures. Participate in audit, quality improvement initiatives, and service evaluation activities. Take responsibility for managing own workload, performance, and professional development.
