Summary
Clinical responsibilities Provide face-to-face, telephone and other appropriate consultations, assessing, diagnosing, treating and managing patients presenting with acute, long-term and complex conditions. Undertake appropriate clinical examinations, investigations, referrals and follow-up, arranging onward care where required. Prescribe safely and effectively in line with current evidence, local formularies, medicines optimisation guidance and antimicrobial stewardship principles. Develop and review care plans with patients, supporting shared decision-making, self-management, preventative healthcare and health promotion. Recognise and respond appropriately to clinical risk, deteriorating patients, safeguarding concerns and urgent or emergency presentations. Undertake home visits where clinically appropriate and in accordance with practice arrangements. Maintain accurate, clear, contemporaneous and confidential clinical records using the practice clinical system. Clinical administration Complete all self-generated clinical administration arising from consultations, including referrals, prescriptions, tasks, correspondence and follow-up, accurately and within agreed timescales. Undertake an appropriate share of pathology and laboratory results, reviewing and acting on results safely and promptly. Participate in the practice arrangements for Electronic Prescription Service (EPS) prescription signing. Review and act on hospital correspondence and other clinical documents allocated to the postholder. Support the appropriate management of workflow and communicate clearly with patients and colleagues regarding agreed actions and follow-up.
