Responsibilities
Conduct comprehensive assessments of individuals and families to identify needs and risks.
Develop and implement personalized care plans in collaboration with clients and their support networks.
Provide direct social work interventions to vulnerable clients in local communities.
Work collaboratively with health, education, and other social care professionals to coordinate services.
Advocate for clients’ rights and access to appropriate resources and support.
Maintain accurate and up-to-date case records and documentation in line with statutory requirements.
Participate in multi-agency meetings, case conferences, and reviews to ensure effective service delivery.
Manage caseloads efficiently while prioritizing client safety and well-being.

End Time: 17:00
Pay Per Hour: £22.21
Location: Kingston Upon Thames
