Description
We are seeking a dedicated and compassionate Hospitals Social Worker (PO3) who is Social Care Qualified to join our multidisciplinary team. In this critical role, you will provide essential social work services within a hospital setting, supporting patients and their families through challenging times with empathy and professionalism. You will work collaboratively with healthcare professionals to assess patients' social needs, coordinate care plans, and facilitate access to community resources and support systems. Your expertise will be vital in addressing complex social issues that impact patients' health and wellbeing, such as mental health, housing, financial difficulties, and safeguarding concerns. Responsibilities
Conduct comprehensive psychosocial assessments of patients admitted to the hospital.
Develop and implement individualized care plans in collaboration with medical and nursing staff.
Provide counseling and emotional support to patients and their families during hospitalization.
Coordinate discharge planning to ensure a smooth transition from hospital to home or other care settings.
Liaise with external agencies and community resources to facilitate ongoing support for patients.
Identify and address safeguarding concerns, ensuring patient safety and adherence to legal requirements.
Maintain accurate and confidential case records and documentation in compliance with professional standards.
Hours Per Week: 36
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £22.21
Location: South London
