About the Role

We are seeking an experienced and motivatedPatient Safety Incident Investigatorto join our Goodall Division. This is an exciting opportunity to play a pivotal role in improving patient safety, driving learning, and shaping high‑quality care across a diverse range of services.

You will lead systems‑based, high‑quality investigations under the Patient Safety Incident Response Framework (PSIRF), ensuring that learning—not blame—drives improvement. As stated in the JD, the postholder “ensures that investigations are timely, compassionate, methodologically robust, and focused on learning and improvement rather than blame.”

What You’ll Be Doing

• Leading and coordinating PSIRF‑aligned patient safety investigations across the Division.

• Applying systems‑based methodologies such as SEIPS, Human Factors, and Just Culture.

• Conducting interviews, evidence reviews, timelines, and systems analysis.

• Producing high‑quality investigation reports with clear findings and proportionate recommendations.

• Ensuring compassionate, transparent engagement with patients, families, and carers.

• Supporting staff involved in incidents and promoting psychological safety.

• Presenting findings at governance meetings and contributing to Trust‑wide learning.

• Working with clinical and operational teams to embed sustainable safety improvements.

• Providing training and coaching on investigation methods and systems thinking.

• Supporting wider Quality Governance activity when required, including tracking actions and identifying themes.

• Deputising for the Head of Quality Governance when needed.
