Summary
What Youll Be Doing Leading and coordinating PSIRFaligned patient safety investigations across the Division. Applying systemsbased methodologies such as SEIPS, Human Factors, and Just Culture. Conducting interviews, evidence reviews, timelines, and systems analysis. Producing highquality investigation reports with clear findings and proportionate recommendations. Ensuring compassionate, transparent engagement with patients, families, and carers. Supporting staff involved in incidents and promoting psychological safety. Presenting findings at governance meetings and contributing to Trustwide learning. Working with clinical and operational teams to embed sustainable safety improvements. Providing training and coaching on investigation methods and systems thinking. Supporting wider Quality Governance activity when required, including tracking actions and identifying themes. Deputising for the Head of Quality Governance when needed.
