Job Summary The Advanced Nurse Practitioner is an autonomous practitioner responsible for the assessment diagnosis treatment and management of patients presenting with acute illness minor illness long term conditions and complex healthcare needs. The postholder will work independently and collaboratively with the wider multidisciplinary team to deliver evidence based care and improve patient outcomes. The role includes clinical leadership service development health promotion education and quality improvement activities. Main Duties and ResponsibilitiesClinical Responsibilities Assess diagnose and treat patients presenting with acute and chronic conditions. Undertake comprehensive clinical assessment and examination. Formulate differential diagnoses and management plans. Independently prescribe within scope of professional practice. Manage patients with long term conditions. Provide same day and urgent care appointments. Undertake home visits where appropriate. Refer patients to secondary care and other services when clinically indicated. Request and interpret diagnostic investigations. Deliver evidence based patient centred care. Participate in vaccination and immunisation programmes. Support health promotion and disease prevention initiatives. Provide advice and education to patients and carers. Maintain accurate and contemporaneous clinical records. Participate in safeguarding processes and act in accordance with local policies. Leadership and Management Provide clinical leadership within the nursing team. Support service development and innovation. Participate in clinical audit and quality improvement activities. Contribute to the achievement of practice and primary care network objectives. Support the development and supervision of other members of the clinical team. Participate in multidisciplinary team meetings. Education and Development Maintain professional knowledge and competence through continuing professional development. Participate in mandatory and statutory training. Support and mentor students trainees and junior colleagues where appropriate. Share learning and best practice across the team. Communication Communicate effectively with patients carers colleagues and external agencies. Provide clear and sensitive information to support informed decision making. Maintain effective working relationships within the multidisciplinary team. Act as an advocate for patients and promote patient centred care. Information Governance and Confidentiality Maintain confidentiality at all times. Comply with Data Protection legislation and practice policies. Ensure accurate and secure management of patient information. Infection Prevention and Control Adhere to practice infection prevention and control policies. Complete all mandatory infection control training. Promote safe working practices to reduce the risk of infection. Professional Responsibilities Maintain current Nursing and Midwifery Council registration. Work within the NMC Code and scope of professional practice. Demonstrate accountability for professional decisions and actions. Promote equality diversity and inclusion in all aspects of the role. Uphold the values and objectives of the practice. General The postholder may be required to undertake other duties appropriate to the role and level of responsibility to support the effective operation of the practice.
