Summary

We are looking for an experienced and enthusiastic Registered General Practice Nurse (GPN) to join our friendly and supportive clinical team. This is a varied general practice nursing role within a busy and progressive practice. We are particularly interested in candidates with strong experience in long-term condition management, alongside the broader skills required to work effectively as a General Practice Nurse. The successful candidate will work autonomously within their professional scope of practice while working closely with our GPs, nursing team, HCAs, pharmacists, care coordinators and wider PCN multidisciplinary team. You will also have the opportunity to contribute to the practice's wider work around long-term condition management, infection prevention and control, clinical governance, quality improvement, QOF and service development. You will not be expected to lead these areas but will be an important part of the team contributing to their continued development. About Knoll Medical Practice Knoll Medical Practice is a progressive, forward-thinking GP practice serving approximately 14,600 patients in Orpington, Kent. We are a team of just over 40 staff, working collaboratively across our clinical, administrative and wider multidisciplinary teams, and are part of Orpington Primary Care Network (PCN). Key responsibilities Clinical care Providing high-quality, evidence-based nursing care to the practice population. Independently assessing, treating and reviewing patients within your professional competence and scope of practice. Undertaking long-term condition reviews and supporting proactive management of patients with chronic disease. Managing patients with conditions including diabetes, COPD, asthma, cardiovascular disease and hypertension. Supporting prevention, health promotion and lifestyle interventions. Providing treatment room services including wound care, dressings and ECGs. Delivering adult and childhood immunisations and vaccinations. Undertaking cervical screening where appropriately trained and accredited. Providing appropriate assessment and management of acute presentations within competence. Recognising deteriorating or seriously unwell patients and escalating appropriately. Working within relevant PGDs, protocols, clinical guidelines and agreed pathways. Long-term condition management Delivering high-quality annual reviews and ongoing management of patients with long-term conditions. Supporting the practice in achieving appropriate clinical outcomes and QOF requirements. Identifying patients requiring additional monitoring, intervention or onward referral. Supporting patients to understand and manage their conditions effectively. Working collaboratively with GPs, pharmacists, PCN colleagues and secondary care services. Contributing to the development and improvement of long-term condition pathways within the practice. Infection Prevention and Control Supporting the practice's infection prevention and control arrangements. Following and promoting appropriate IPC procedures and standards within clinical areas. Participating in IPC audits and associated action plans. Ensuring appropriate cleaning, decontamination and safe use of clinical equipment. Maintaining appropriate standards around clinical waste and sharps management. Keeping up to date with relevant IPC guidance. Supporting the practice's IPC lead and wider team with the implementation and maintenance of infection-control standards. Clinical governance and quality improvement Participating in clinical audits and quality improvement projects. Contributing to significant event reviews and shared learning where appropriate. Supporting the practice to maintain high standards of clinical governance. Maintaining accurate and appropriate clinical records and coding. Contributing to QOF, enhanced services and other relevant contractual requirements. Identifying opportunities to improve patient care, clinical outcomes and patient experience. Supporting the practice with CQC requirements and evidence where appropriate. Participating in nursing and wider clinical team meetings. Teamwork and professional development Working collaboratively with the wider practice and PCN multidisciplinary team. Supporting the induction and development of colleagues where appropriate. Supporting students and trainee healthcare professionals where suitably qualified and experienced. Maintaining your own professional development and clinical competencies. Sharing knowledge and good practice with colleagues. Contributing to a positive, supportive and respectful team environment. Digital and service improvement Making effective use of EMIS Web and other digital systems. Using clinical recalls, population health information and practice data to support proactive patient care. Embracing digital technology and new ways of working. Contributing ideas to improve clinical services, patient experience and staff workflow. Being open to change and willing to adapt as primary care continues to evolve.
