Summary
Job Purpose The Practice Nurse will work as part of the practices multidisciplinary team to provide high-quality, evidence-based nursing care to the practice population. The post holder will deliver appropriate treatment, screening, health promotion, education and clinical advice within their professional competence and scope of practice. The Practice Nurse will develop and manage individualised care programmes for patients with long-term conditions and specific health needs, contribute to preventative health programmes, participate in clinical audits and quality improvement initiatives, and support the delivery of practice targets and contractual requirements. The post holder may be required to undertake additional duties or adapt existing responsibilities to support the effective and efficient running of the practice. The Partners and Practice Manager reserve the right to reasonably amend or redistribute duties in response to changing service requirements. Key Responsibilities1. Clinical and Treatment Room Duties The Practice Nurse will provide a broad range of clinical services, including: Wound assessment, treatment and ongoing wound management. Removal of sutures and clips. Administration of routine injections and vaccinations. Cervical screening (cytology) in accordance with national guidance. Taking and recording blood pressure, pulse, oxygen saturation and other clinical observations. Supporting patients with inhaler technique and respiratory self-management. Ear care where appropriately trained and within practice policy. Assisting GPs with minor surgery and other clinical procedures. Undertaking clinical assessments and providing appropriate treatment and advice within scope of practice. Requesting appropriate pathology investigations, such as urine cultures and swabs, in accordance with practice protocols. Reviewing and acting on investigation results within agreed clinical protocols and scope of practice. Providing appropriate follow-up and safety-netting advice. 2. Long-Term Condition Management The post holder will contribute to the assessment, monitoring and management of patients with long-term conditions, including: Diabetes. Asthma. COPD. Hypertension. Cardiovascular disease. Other chronic conditions relevant to the practice population. Responsibilities will include: Conducting structured reviews and assessments. Monitoring clinical indicators and outcomes. Developing and reviewing individual care plans. Providing self-management advice and education. Identifying deterioration and escalating concerns appropriately. Making appropriate referrals to other members of the multidisciplinary team and external services. 3. Health Promotion and Preventative Care The Practice Nurse will support the development and delivery of preventative health and health promotion programmes, including: Cervical screening and appropriate follow-up. Childhood and adult immunisation programmes. Travel vaccinations and travel health advice. Cardiovascular disease prevention. Diabetes prevention and education. Weight management and physical activity advice. Sexual health and contraception advice within competence. Supporting patients with appropriate self-examination and awareness advice. Promoting patient access to reliable health information and self-management resources. 4. Immunisation and Vaccination Programmes The Practice Nurse will: Administer childhood, adult, travel and other appropriate vaccinations within competence. Support the planning and delivery of vaccination programmes. Participate in recall and invitation systems. Ensure vaccinations are correctly documented and coded. Maintain appropriate knowledge of current vaccination schedules and national guidance. Maintain and monitor the vaccine cold chain in accordance with national guidance. Report and appropriately manage any cold-chain incidents. Ensure appropriate stock levels and expiry-date checks are maintained. Support the practice in meeting relevant vaccination targets. 5. Infection Prevention and Control The post holder will be responsible and accountable for maintaining high standards of infection prevention and control within their clinical practice. Responsibilities include: Following the practice infection prevention and control policies and procedures. Ensuring clinical rooms and treatment areas are maintained to the required standards. Maintaining appropriate standards of cleanliness, decontamination and clinical waste management. Ensuring equipment is appropriately cleaned, maintained and stored. Maintaining up-to-date infection prevention and control knowledge and training. Identifying and reporting any infection control concerns to the Practice Manager or appropriate clinical lead. Supporting wider staff awareness of infection prevention and control requirements. 6. Emergency Equipment and Medicines The Practice Nurse will support the safe management of clinical emergency equipment and medicines, including: Checking emergency equipment and emergency medicines in accordance with practice procedures. Monitoring expiry dates and stock levels. Ensuring used or expired medicines and equipment are replaced promptly. Supporting the maintenance of emergency bags and equipment. Reporting any concerns or deficiencies immediately to the appropriate manager or clinician. 7. Advice, Signposting and Referrals The post holder will: Provide appropriate telephone and face-to-face clinical advice. Signpost patients to appropriate services and reliable sources of health information. Make appropriate referrals to primary, secondary and community health services. Work collaboratively with other healthcare professionals to ensure continuity of care. Provide appropriate safety-netting advice and escalate concerns where required. 8. Safeguarding All staff have a responsibility to safeguard and promote the welfare of children, young people and adults at risk. The post holder will: Complete safeguarding training appropriate to their role. Understand and comply with the practice safeguarding policies and procedures. Recognise and appropriately respond to safeguarding concerns. Record and report safeguarding concerns accurately and promptly. Work collaboratively with the practice safeguarding team and external agencies where appropriate. 9. Clinical Governance, Quality and Audit The Practice Nurse will contribute to the practices clinical governance and quality improvement programme by: Supporting the achievement of contractual and quality targets. Reviewing clinical outcomes and identifying opportunities for improvement. Supporting the development and review of clinical protocols. Participating in significant event reviews and learning where appropriate. Supporting the implementation of evidence-based practice and national guidance. Identifying and reporting clinical risks and incidents appropriately. 10. Team Working and Leadership The post holder will work collaboratively with GPs, nurses, healthcare assistants, pharmacists, administrative staff and other members of the multidisciplinary team. Responsibilities include: Contributing positively to the wider practice team. Attending relevant clinical and practice meetings. Sharing relevant clinical information and updates with colleagues. Supporting the development of nursing services within the practice. Supporting and mentoring healthcare assistants, trainee nurses and junior members of the nursing team. Sharing knowledge, skills and good practice with colleagues. Developing leadership skills and taking responsibility for areas of clinical interest where appropriate. Building effective working relationships with primary, community and secondary care professionals. 11. Administration and Organisation The post holder will contribute to the effective organisation of nursing services by: Maintaining accurate and timely clinical documentation. Ensuring appropriate coding and recording of clinical activity. Monitoring and responding to relevant clinical tasks and results. Maintaining appropriate clinical stock and equipment. Supporting the achievement of practice targets. Working within agreed practice protocols and procedures. Supporting the development and implementation of practice development plans. 12. Professional Development The Practice Nurse will take responsibility for maintaining and developing their professional knowledge and skills. This includes: Participating in annual appraisal and personal development planning. Maintaining professional registration and meeting NMC requirements. Completing mandatory and role-specific training. Attending relevant courses, study days and clinical education sessions. Participating in clinical supervision. Developing teaching, mentoring and leadership skills. Making full use of available learning and development opportunities. Maintaining professional links with other Practice Nurses and relevant professional networks to avoid professional isolation. General Responsibilities The post holder will: Work within the scope of their professional competence and NMC requirements. Maintain patient confidentiality at all times. Comply with UK GDPR, information governance and practice confidentiality policies. Comply with relevant health and safety legislation and practice procedures. Take reasonable care of their own health and safety and that of others. Report incidents, risks and concerns through the appropriate practice procedures.
