Description
We are seeking a compassionate and dedicated Adults Social Worker (CAP) to join our multidisciplinary team committed to improving the lives of adults in our community. The Adults Social Worker (CAP) will advocate for clients' rights and needs, helping them navigate complex social systems and access necessary resources. Responsibilities
Conduct comprehensive assessments of adults in need of social care support.
Develop and implement individualized care and support plans.
Coordinate services with healthcare professionals, community organizations, and families.
Provide guidance and advocacy to empower clients in making informed decisions.
Monitor and review the progress of clients' care plans regularly.
Maintain accurate and timely records in accordance with data protection standards.
Stay up-to-date with relevant legislation, policies, and best practices in adult social care.
Work collaboratively within a multidisciplinary team to deliver holistic support.
Hours Per Week: 37
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £21.31
