Summary
Ideally working between 4 and 7 sessions per week. Provide high-quality, patient-centred general medical services as part of a multidisciplinary primary care team. Undertake clinical assessment, diagnosis and management of patients with a wide range of acute, urgent, chronic and complex health needs, ensuring safe and effective clinical decision-making. Support the delivery of same-day and urgent care, including seeing patients with acute problems and undertaking clinical triage of patient requests through Anima Total Triage during allocated sessions. Support continuity of care and the management of patients with long-term conditions, frailty and complex health needs. Contribute to the practice's New Models of Care work, supporting proactive and coordinated care for patients who are frail, have multiple or complex long-term conditions, are approaching the end of life or make frequent use of secondary care services. Work collaboratively with GPs, nursing staff, pharmacists, paramedics, healthcare assistants, administrative colleagues and the wider multidisciplinary team to deliver safe and effective services. Participate in multidisciplinary discussions and care planning, including working with colleagues across the East Northants Neighbourhood and, where appropriate, specialists from hospital and community services. Maintain accurate and contemporaneous clinical records and make appropriate referrals, investigations and follow-up arrangements. Contribute to quality improvement and the ongoing development of services within the practice, where appropriate.
