Summary

To provide high-quality, patient-centred nursing care within the practice, promoting health, preventing disease and supporting the management of acute and long-term conditions. The post holder will lead the practice's diabetes service and childhood immunisation programme, ensuring the delivery of safe, effective, evidence-based care in accordance with national guidance and practice policies. Main Duties and Responsibilities Clinical Responsibilities Provide holistic nursing care to patients of all ages. Assess, plan, implement and evaluate individual patient care. Undertake chronic disease management clinics in line with national guidance and local protocols. Perform cervical cytology screening. Undertake NHS Health Checks and health promotion activities. Administer adult and childhood vaccinations and immunisations. Undertake wound care, dressings, suture and clip removal. Perform venepuncture, ECGs, blood pressure monitoring, urinalysis and other routine clinical procedures. Provide lifestyle advice on smoking cessation, healthy eating, weight management and exercise. Recognise and respond appropriately to medical emergencies within the practice. Lead the delivery and development of the practice diabetes service. Undertake comprehensive annual diabetes reviews. Monitor glycaemic control, blood pressure, lipid management and cardiovascular risk. Perform diabetic foot assessments and ensure appropriate referrals. Support patients in self-management through education and lifestyle advice. Review medication within agreed competencies and escalate concerns to GPs where appropriate. Ensure timely patient recall and follow-up. Monitor Quality and Outcomes Framework (QOF) performance relating to diabetes. Work collaboratively with GPs, community diabetes teams and secondary care services. Maintain up-to-date knowledge of national diabetes guidance and implement best practice. Lead the practice childhood immunisation programme. Administer vaccines safely in accordance with the national immunisation schedule and Patient Group Directions (PGDs). Ensure robust call and recall systems are maintained. Monitor immunisation uptake and identify opportunities to improve coverage. Provide evidence-based information and reassurance to parents and carers. Ensure safe vaccine storage, cold chain compliance and stock management. Maintain accurate clinical records and submit data as required. Report and manage vaccine incidents in accordance with national guidance. Maintain accurate, contemporaneous clinical records. Participate in clinical audit and quality improvement projects. Maintain NMC registration and professional revalidation requirements. Participate in annual appraisal and mandatory training. Maintain confidentiality in accordance with GDPR and practice policies. Adhere to safeguarding, infection prevention and control, health and safety and information governance requirements. Contribute to CQC compliance and continuous service improvement. Communication and Team Working Work collaboratively with GPs, nurses, administrative staff and the wider multidisciplinary team. Develop positive working relationships with community and secondary care colleagues. Participate in practice meetings and contribute to service development. Professional Responsibilities Work within the NMC Code of Professional Standards. Maintain professional competence through continuing professional development. Work within own scope of practice and seek support when appropriate. Promote equality, diversity and inclusion in all aspects of care. General Responsibilities Comply with all practice policies and procedures. Undertake any other duties appropriate to the role and level of responsibility as requested by the Partners or Practice Manager.
