Summary

The postholder will work as part of a community nursing service working across the Fylde coast, undertaking nursing duties in patients’ homes or clinic settings.

They will provide safe, effective, individualised care to patients and to give support to families and service users requiring scheduled and unscheduled care interventions.

The role includes planning, implementing and evaluating programmes of care, initiating changes to care and escalation, if required, to the senior community nurse and the wider team.

They will  monitor the condition and health needs of patients on a continual basis in partnership with colleagues, families and carers, referring to others for reassessment, when required.

They will assist the Senior Nurse in the day-to-day management of the team in consultation with and deputising for the senior nurse in their absence following in house training.

To deliver a commissioned service to housebound patients and clinic patients who fall under the agreed service specification.

• Deliver holistic, person-centred nursing care

• Actively engage with individuals, their families and/or carers and contribute to care planning, by establishing their needs, wishes, preferences and choices and incorporating these into care planning.
