Summary

JOB SUMMARY: The postholder will work as part of a community nursing service working across the Fylde coast,overseeing/supervisingthe provision ofnursinginterventionsin patients homes or clinic settings. To manage a caseload of patients withhighly complexneeds using evidence base and patient centred principles to assess, plan, implement and evaluate interventions. To provide safe, effective, individualised care to patients and to give support to families and service usersrequiring scheduled and unscheduled care interventions. Work in collaboration with colleagues and key stakeholders, ensure the delivery of a co-ordinated multi agency service. This includes planning, delivering, evaluating and development of the services for which they are responsible. Tofacilitate themonitoringofthe health needs of patients on a continual basis in partnership with colleagues, families and carers,and referralsto others for reassessment, when required. To ensure delivery of improved condition management strategies, incorporating multi professional groups through the standardisation of assessment and referral processes and to ensure access to associated support services throughappropriate,timelyreferral. Ensure that all relevant informationis cascaded to all staff on a regular and timelybasis, providingthe opportunity for feedback as appropriate. Attend appropriate meetings/ events as required and provide the necessary feedback. Undertake supervision ofidentifiedstaff, including theirpersonal development plan,andappraisal completion. Holdadefined clinical caseload (ifappropriate), co-ordinateadmissionsand discharges, for patients registered to specific GP surgeries. Oversee the assessment,planningandimplementation of patient care.Ensuring the maintenance andtimelycompletion ofassociatedclinicalrecords. To report andassistin investigation of patient complaintsand/or relevant incidents and implement lessons learned action plans following Trust policy. Provide leadership and operational management ofa communitynursingteam, supporting,supervisingand managing staff; in conjunction with the Team Leader. Provide direct patient careto patient with complex needsand health concerns. Contribute to the development of nursing serviceswithinthedivision. Provide clinical supervision formembers of the nursing team. Responsibilityfor the leadership andimplementationof servicesincluding managing issues relating to performance managementand compliance with Care Quality Commission standards.
