Summary

The postholder will work as part of a community nursing service working across the Fylde coast, overseeing/supervising the provision of nursing interventions in patients’ homes or clinic settings.

Support quality improvement and optimisation of resources to improve services and promote equity in access to services.

Participate inappraisal process and Personal Development Plans.

EDUCATIONAL

Undertake the role of mentor and preceptor.

Encourage reflective practice, use of professional portfolios, MURRS documents, and critical incident analysis.

RESEARCH AND DEVELOPMENT

Apply critical analytical skills in research/audit/service improvement context.

Adhere to ethical, legal, governance and quality assurance frameworks that pertain to research development and innovation.

JOB SUMMARY:

The postholder will work as part of a community nursing service working across the Fylde coast, overseeing/supervising the provision of nursing interventions in patients’ homes or clinic settings.

To manage a caseload of patients with highly complex needs using evidence base and patient centred principles to assess, plan, implement and evaluate interventions.

To provide safe, effective, individualised care to patients and to give support to families and service users requiring scheduled and unscheduled care interventions.

Work in collaboration with colleagues and key stakeholders, ensure the delivery of a co-ordinated multi agency service. This includes planning, delivering, evaluating and development of the services for which they are responsible.

To facilitate the monitoring of the health needs of patients on a continual basis in partnership with colleagues, families and carers, and referrals to others for reassessment, when required.

To ensure delivery of improved condition management strategies, incorporating multi professional groups through the standardisation of assessment and referral processes and to ensure access to associated support services through appropriate, timely referral.

Ensure that all relevant information is cascaded to all staff on a regular and timely basis, providing the opportunity for feedback as appropriate.

Attend appropriate meetings / events as required and provide the necessary feedback.

Undertake supervision of identified staff, including their personal development plan, and appraisal completion.

Hold a defined clinical caseload (if appropriate), co-ordinate admissions and discharges, for patients registered to specific GP surgeries.

Oversee the assessment, planning and implementation of patient care. Ensuring the maintenance and timely completion of associated clinical records.

To report and assist in investigation of patient complaints and/or relevant incidents and implement lessons learned action plans following Trust policy.

Provide leadership and operational management of a community nursing team, supporting, supervising and managing staff; in conjunction with the Team Leader.

Provide direct patient care to patient with complex needs and health concerns.

Contribute to the development of nursing services within the division.

Provide clinical supervision for members of the nursing team.

Responsibility for the leadership and implementation of services including managing issues relating to performance management and compliance with Care Quality Commission standards.
