Summary

You will provide high-quality, person-centred care, enabling the individual to remain safe, comfortable and independent in their own home.

Duties will include:

Delivering high-quality personal care and support

Supporting individuals with complex healthcare needs

Bowel management (essential)

Assisting with activities of daily living (ADLs)

Supporting mobility and positioning where required

Monitoring and accurately recording care provided

Working in accordance with individual care plans, risk assessments and clinical guidance

Promoting dignity, choice, independence and wellbeing

Maintaining effective communication with family members and healthcare professionals
