    We have an exciting opportunity for a full-time Band 6 Speech and Language Therapist to join the Camden Community Team within the Camden Neurology and Stroke Service (CNSS). This post is an excellent opportunity for an existing Band 6 therapist, or an experienced Band 5 looking to develop their specialist skills in working with adults with acquired communication and swallowing disorders. The Camden Community Team provides assessment, rehabilitation and management for adults with a wide range of neurological conditions, including stroke, progressive neurological diseases and other acquired neurological disorders. We support service users in their own homes, community settings and clinics across Camden. As part of CNSS, the successful candidate will work closely within a well-established multidisciplinary team and will have opportunities to develop skills across a broad range of pathways, including Palliative Care, Camden Integrated Community Healthcare, the Rapid Access Service and Virtual Ward. The role offers the opportunity to deliver person-centred care, contribute to service development and work collaboratively with colleagues across health and social care services. JOB SUMMARY: 1. To provide a specialist assessment and treatment service to adults living in the community with acquired communication and/or swallowing difficulties. • The caseload is comprised of adults with neurological diagnoses, including stroke, and other long-term conditions and palliative presentations (e.g. COPD, dementia). • Patients are seen in a variety of settings, including their own homes and residential care homes. • Community visits to patients may be made on foot, public transport, motorcycle, bicycle or car. 2. To independently manage (with support) a complex caseload using evidence-based clinical reasoning, critical thinking, reflection and analysis to support assessment and intervention. 3. To work flexibly with all other members of the team to provide an integrated rehabilitation service to patients in accordance with the philosophy and operational policies of the team. 4. To use a client-centred, goal-orientated rehabilitation and disability management approaches in line with local and national clinical guidelines. 5. To be actively involved in CPD and to strive to implement high quality evidence-based practice. 6. To be actively involved in clinical governance including service development and quality improvement projects. To act as an advocate on behalf of clients and carers by adopting a client-centred approach to support meaningful participation in life roles within the local communities. 8. To ensure that the client and/or their carer have an understanding of treatment protocols and give informed consent to examination and treatment. 9. To ensure that the patient understands the clinical reasoning process and the management plan. 10. To modify the patient’s therapy treatment/management plan as appropriate following ongoing re-assessment and evaluation of progress. 11. To manage a caseload of clients with complex needs with supervision and guidance as necessary from senior staff. To successfully monitor, evaluate and review Speech & Language Therapy treatment/interventions within a multi-disciplinary framework and to modify as necessary taking a key worker role as indicated. 12. To use knowledge of other multi-disciplinary roles and external agencies to ensure effective referral management and achieve best client care. 13. To have the ability, knowledge and skills required for moving and handling people in inpatient and community environments (as required for swallow assessments). 14. To ensure that all documentation is accurate, contemporaneous and meets the documentation policy of the Trust. 15. To prescribe, delegate and monitor the work of rehabilitation assistants as required. 16. To manage clinical risk in own caseload and ensure that rehabilitation assistants and students comply e.g manual handling, risk assessment and local policy regarding lone working. 17. To contribute to the efficient, day-to-day, running of the team. 18. To maintain case notes and write reports outlining recommendations for discharge, independently or jointly with other professions. These are communicated to external agencies e.g. GPs, Consultants, Social Services. Professional:1. To keep up to date with Speech & Language Therapy professional registration and clinicalpractice through CPD activities and be able to demonstrate a commitment to CPD as required of the HCPC. 1. To develop a specialist knowledge base in rehabilitation and disability management, assessment, clinical reasoning and management skills. 1. To evaluate your practice through audit and outcome measures. 4. To access library services, medical databases and the Intranet to further develop clinical practice and service initiatives. 5. To develop links and communicate with other members of the multidisciplinary team. 1. To be professionally and legally accountable for all work undertaken. Management: 1. To ensure that accurate and evaluative records of patient care are maintained in accordance with the Trust/ Service/Speech & Language Therapy service policies. 1. To manage a clinical case load supported by senior staff. 1. To participate in the Trust’s appraisal and supervision schemes and contribute to the appraisal of rehabilitation assistants and Speech & Language Therapists with support from senior staff. 4. To ensure that all equipment defects, accidents, incidents and complaints are reported to the line manager immediately and that appropriate action is taken following Trust and Service policies. 1. To supervise junior members of staff, rehabilitation assistants and volunteers as required. To delegate administrative and clinical tasks appropriate to their competencies. 1. You will be working within the Hillingdon Probation office working alongside Probation practitioners and other key multi agency staff involved with the young adult caseload. You will take the lead on setting up and delivering all aspects of the Speech and Language Therapy Service within Hillingdon Probation Services. We are recruiting for 2 x Band 6RMNs/OTs/Social Workers to work in the Boroughs of Kensington and Chelsea and/or Westminster The Westminster and Kensington & Chelsea Older Adults and Healthy Ageing Community Mental Health Teams are developing a new primary care pathway to provide early access to support for older adults who are experiencing mental health difficulties. In line with the mental health transformation agenda, the primary care pathway will focus on the integration and development of neighbourhood-based care alongside primary care, local authorities and third sector providers. Furthermore, the service will focus on early intervention, working proactively with community partners to provide interventions to meet needs, reduce risks and prevent escalation by supporting service users to remain well in the community, thereby reducing the need for secondary care interventions and admissions to hospital. As a member of the OA PCN Team, the post holder will be a team member working in co-operation with other disciplines, statutory, voluntary and private agencies as required. Amongst other tasks, the post holder is expected to work as a clinician to provide mental health nursing, occupational therapy and social work interventions, provide specific assessments and treatment interventions to service users referred to the OA PCN team,demonstrable experience of working with older adults/ those with complex physical and mental health needs including functional and organic illness, an ability to work flexibly and collaboratively across local environments and strong interpersonal skills,manage a caseload as needed, including providing access and /or deliver a broad range of psychosocial interventions for individual patients, carry out robust risk assessments and care planning. The primary care pathways will sit within the Westminster and Kensington & Chelsea Older Adults and Healthy Ageing Community Mental Health Teams. These are well established dynamic multidisciplinary services providing mental health support to older adults in Westminster and Kensington & Chelsea with functional disorders and dementia. They are co-located with the Westminster City and Kensington and Chelsea Council mental health team, offering a holistic approach to care. The Westminster team is also involved in the CNWL Open Dialogue project, which promotes person-centred recovery-oriented care. The teams operate using a case management, intervention-led and recovery-focused model, offering multidisciplinary assessments and interventions for service users with complex needs, as well as their families and carers. Regular multidisciplinary meetings facilitate case discussions, initial assessments, and reflective practice. A key feature of the services is the integrated older adults home treatment team, which delivers intensive support, crisis management, admission prevention, and helps ensure timely hospital discharges. The CMHT and HTT teams collaborate closely, offering a responsive and flexible service to meet the needs of service users. Coordination with older adults’ inpatient services allows smooth transitions of care. Clinical: 1. To triage referrals and allocate work to junior staff as appropriate. 1. To provide a comprehensive mental health assessment for service users with complex needs which can include anxiety, depression, dementia, physical health co-morbidities; health anxieties; drug and alcohol misuse. 1. To formulate, implement and review individualised Recovery Care Plans with patients and their carers that aims to address their psychological, physical & social needs. 1. To assess current risks and identify how the risk and service users’ mental health needs would best be met. 1. To provide specialist nursing/OT/social worker expertise and collaborative support, contributing to assessment, care planning, interventions and ongoing reviews. 1. To regularly monitor and evaluate the therapeutic value of interventions and to adjust treatment plans accordingly. 1. To build and maintain therapeutic relationships with service users and carers and involve them in drawing up care plans. 1. To provide training and advice to carers, service users and colleagues on lifestyle changes and adaptations to the service users’ social and physical environment. 1. To ensure support is available to relatives and carers as appropriate and to refer on to relevant agencies as required. 1. To support and practice Open Dialogue and dialogical practice within the service if relevant. Communication: 1. To communicate effectively with service users, carers/relatives, other professionals and agencies, providing assessment, support and information as required, whilst observing the boundaries of service user confidentiality. 2. To participate in relevant PCN based multi-disciplinary and team meetings, and case conferences. 3. To support the delivery of specialist training, advice, and ongoing support to PCN colleagues to enhance their knowledge and confidence in recognising, assessing, and responding to the mental health needs of older adults 4. To promote service user involvement in service delivery where possible. Documentation: 1. To ensure that accurate and up-to-date written and electronic records are maintained in accordance with Trust policy. 2. To ensure the timely completion of required statistical information. 3.
