    Job Title: - Coaching Support Coordinator Purpose of Job: - to coordinate a support package to Links members as described below. Responsible to: - Trustees / General Manager Salary: - £24,652 per annum, pro rata. The contract will be fixed term for 1 year, dependent on ongoing funding. Main Tasks of the Job To provide and coordinate support within the project as required: - ● To process new referrals. ● To arrange visits, and an induction process for new referrals. ● To undertake and coordinate at least quarterly Personal Goal Plans and risk assessment reviews with members. ● To support members individually in times of need and signpost or refer to other agencies as appropriate. ● To maintain contact with members who have failed to attend and encourage/facilitate reintegration into the project. ● To drive forward new member initiatives and support growth and development of services that enhance member experience. ● To coordinate quarterly member external trips. ● To capture member case studies and journeys for reporting, awareness and publicity. ● To work closely with the staff and supporting organisations to ensure the member has received the correct support. ● To ensure all paperwork and recording is commensurate with Links procedures. ● To keep open lines of communication with other Links staff; to ensure that staff are kept informed, and records are made, of changes or developments regarding members. ● To work in partnership with voluntary and statutory organisations to ensure that the service provided is one that is complementary to other services available to members. Other; ● To undertake initial induction. ● To undergo quarterly supervision sessions. ● To adhere to existing Links working practices, methods, procedures, undertake relevant training and development activities and to respond positively to new and alternative systems. ● To attend team meetings and planning groups as required. ● To maintain confidentiality, observe data protection and safeguarding associated guidelines. ● To carry out any other reasonable duties and responsibilities within the overall function, in line with the grading and level of responsibilities of the post.
