Summary

A Care Team Leader is responsible for leading and supporting the care team to deliver high-quality, person-centred care for residents living with dementia.

Key Responsibilities:

Lead and support care staff during shifts.

Deliver and oversee person-centred dementia care.

Administer and monitor medication in line with policies.

Maintain accurate care records and reports.

Support residents with daily living activities and emotional wellbeing.

Liaise with families and healthcare professionals.

Conduct handovers and escalate concerns appropriately.

Ensure health and safety, safeguarding, and regulatory standards are met.

Mentor, supervise, and support the development of care staff.
