Summary
Clinical assessment and triage; referral management; palliative and end of life care; advice and signposting; multidisciplinary and multi-agency working; care coordination; safeguarding; clinical governance; quality improvement; audit; service development; professional development and reflective practice. The role is based at Ashgate Hospice, with some flexibility for hybrid working, and is 37.5 hours per week, Monday to Friday, 9am5pm.
