Summary

Job Description Fixed Term Salaried General Practitioner Contract: Fixed-Term until 31 March 2027 Hours: 3 Sessions per Week Location: Great Homer Street Medical Centre, Liverpool Accountable to: GP Partner and Practice Manager Job Purpose The Salaried GP will provide high-quality, patient-centred primary care services to the registered practice population, delivering care in accordance with the General Medical Services (GMS) Contract, professional standards and evidence-based practice. The postholder will work as part of a multidisciplinary team to assess, diagnose, treat and manage patients across the full spectrum of primary care presentations. The role requires a commitment to delivering safe, effective and compassionate care whilst supporting the practice's focus on improving health outcomes and reducing health inequalities. Key Responsibilities Clinical Duties Provide a full range of primary medical services to registered patients. Undertake face-to-face, telephone and digital consultations as required. Assess, investigate, diagnose and treat patients presenting with acute and chronic conditions. Manage patients presenting with undifferentiated and undiagnosed illnesses. Develop and implement appropriate management plans. Manage patients with long-term conditions in line with local and national guidance. Provide preventative healthcare, screening, health promotion and lifestyle advice. Identify disease risk factors and support early intervention where appropriate. Prescribe in accordance with national and local prescribing guidance. Undertake home visits where clinically indicated. Provide appropriate palliative and end-of-life care in conjunction with community services. Maintain continuity of care where possible. Arrange appropriate referrals to secondary care and other services. Care Planning and Patient Management Develop personalised care plans with patients and carers. Support self-management and patient engagement in their healthcare. Work collaboratively with patients to improve health outcomes. Recognise and respond appropriately to safeguarding concerns involving children and vulnerable adults. Multidisciplinary Working Work closely with practice colleagues and members of the multidisciplinary team. Liaise effectively with community, secondary care and voluntary sector services. Participate in multidisciplinary meetings where appropriate. Support integrated care pathways and collaborative working. Clinical Governance and Quality Maintain accurate and legally compliant patient records. Participate in clinical audit and quality improvement activity. Contribute to the achievement of contractual and quality targets. Participate in significant event reviews and patient safety initiatives. Work within GMC guidance and practice policies. Professional Responsibilities Maintain full GMC registration and inclusion on the NHS England Performers List. Participate fully in appraisal and revalidation. Maintain mandatory training requirements. Participate in continuing professional development activities. Support the educational and training ethos of the practice. Person Specification Essential Qualifications MBBS or equivalent medical qualification. Full GMC Registration with Licence to Practise. Inclusion on the NHS England Performers List. MRCGP or equivalent. Essential Experience Experience of working within UK General Practice. Experience in managing acute and chronic disease. Evidence of multidisciplinary team working. Understanding of clinical governance and quality improvement. Essential Skills Excellent communication and consultation skills. Strong clinical decision-making abilities. Ability to prioritise workload effectively. Good IT and record-keeping skills. Commitment to patient-centred care. Ability to work independently and as part of a team. Desirable Experience of working with socially and clinically complex populations. Experience of managing patients with multiple long-term conditions. Experience using EMIS Web. Knowledge of local healthcare pathways. About the Practice Great Homer Street Medical Centre is a GP training practice based in North Liverpool. The practice is led by one GP Partner and supported by three GPs, alongside an experienced multidisciplinary team. The practice is committed to delivering high-quality primary care services, supporting professional development and providing compassionate, patient-centred care to its registered population. Terms and Conditions Fixed-Term Contract ending 31 March 2027. Three clinical sessions per week. NHS Pension Scheme. Annual leave and study leave in accordance with NHS Terms and Conditions.
