Summary

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare or working on the Staff Bank within these organisations. Nursing and AHP vacancies are also open to Student Nurses and Newly Qualified Nurses/AHPs.

Join our Community Nursing Team and make a real difference to patients' lives by delivering high-quality nursing care to housebound adults aged 18 and over. Our service supports individuals with complex and long-term health conditions, helping them remain safe, well, and independent in their own homes. As a Nurse Associate, you will work autonomously and as part of a multidisciplinary team, providing personalised, holistic care to patients within their chosen environment.

You will undertake home visits, manage a delegated caseload, participate in team handovers, and work collaboratively with colleagues, carers, and partner agencies to ensure seamless patient care. This is a rewarding and varied role that offers the opportunity to build strong therapeutic relationships while supporting patients to achieve the best possible health outcomes.

No two days are the same in Community Nursing. You will deliver a wide range of clinical interventions and assessments, including monitoring and accurately recording vital signs such as blood pressure, respiratory rate, oxygen saturation, temperature, and Glasgow Coma Scale (GCS). You will administer medications via oral, topical, intravenous, subcutaneous, and Controlled Medicines Routes (CMR), undertake venepuncture and peripheral cannulation, and provide care and management of intravenous devices.

Additional responsibilities include male and female catheterisation, bladder and bowel care, wound assessment and dressing management, removal of wound closures and drains, symptom management, and holistic patient assessments. Working closely with the multidisciplinary team, you will contribute to care planning, communicate effectively with other agencies involved in patient care, and support the delivery of safe, compassionate, and person-centred services within the community.

Kent Community Health NHS Foundation Trust is due to integrate with Medway Community Healthcare on 1 October 2026. Following integration and to align our community services to the new Neighbourhood Model of care delivery, there will be a period of consultation which may result in changes affecting some services, teams or roles.
