Summary

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare or working on the Staff Bank within these organisations. Nursing and AHP vacancies are also open to Student Nurses and Newly Qualified Nurses/AHPs.

We are looking for an enthusiastic and motivatedBand 6 Senior Wound Care Nurseto join our dedicatedTissue Viability and Wound Therapy Service.

This is a fantastic opportunity to play a key role in delivering high-quality, evidence-based wound care, supporting colleagues and helping shape the future of wound management services across Medway Community Healthcare.

Your daily activities will be varied to include:

• Assess, treat and monitor patients attending  wound clinics, delivering evidence-based wound care and evaluating treatment outcomes.  

• Conduct comprehensive wound assessments  and develop individualised care plans in collaboration with patients and the 

multidisciplinary team.  

• Provide education, advice and support to patients, families and carers to promote wound healing, self-management and health 

improvement.  

• Communicate effectively with GPs, community services, care homes, hospitals and other healthcare professionals to ensure coordinated patient care.  

• Organise and prepare clinic sessions, prioritising workloads and managing appointments to meet patient needs efficiently 

Kent Community Health NHS Foundation Trust is due to integrate with Medway Community Healthcare on 1 October 2026. Following integration and to align our community services to the new Neighbourhood Model of care delivery, there will be a period of consultation which may result in changes affecting some services, teams or roles.
