Summary

We have an exciting opportunity for Band 6 Mental Health Nurses to join the Older Adults community team based in South Warwickshire. We are a developing team committed to providing excellent patient care, proactively embedding and utilising multidisciplinary roles to provide recovery and outcome focussed interventions for individuals within the service.

We are seeking candidates who have experience of working in mental health settings who have a keen interest in working with older adults, are innovative, dynamic andlooking to develop their clinical skills. They will be able to demonstrate good verbal and written communication skills and exhibit behaviours that reflect Coventry and Warwickshire Partnership Trust vision and values.

A full UK driving licence and access to your own transport is essential in this role.

As a Community psychiatric nurse within the Older adult service your role will include managing a caseload of complex patients presenting with a wide array of mental health diagnoses.

Strong organisational, communication and risk management skills are essential.

• Management of a caseload of patients presenting with complex needs.

• Responsibility for the day-to-day delivery of the service to individual clients and their families and carers via a range of evidence-based interventions. 

• Delivery of specific interventions in line with the treatment pathway.

• Generate options including modification or alternatives to planned intervention to meet clinical need as appropriate.

• Agree course of action with those responsible for performing the interventions including supporting junior staff.

• You may be required to provide supervision for junior nursing staff and support workers in the team.

• Enable service users to choose and take part in activities that are meaningful to them. These may include self care, education, recreational and work activities.

• Participate as part of the multi-disciplinary team implementing, evaluating and reviewing all aspects of the service users' care

• Advise, inform and support on the physical health and wellbeing of individuals (services users, their family, their significant other) in a holistic way.
