Summary

Care Home In Reach Nurse

This is a cross county post, covering Coventry and North and South Warwickshire, within the community Older Adults Team working with functional mental illness and dementia

This post supports our Hospital at Home Pathway which is a 7 day a week service working between the hours of 08.00-20.00

The focus of this position is:

Triage care home referrals- completing a full review at point of triage, to identify needs fully, respond to urgent needs quicker and improve service response time in both routine and urgent referrals .

Care Home Liaison- building relationships with care homes across trust ensuring better communication, relationships and improving outcomes for service users who have complex needs

Reducing admission to both General and Mental Health  Hospital

Better response time to urgent situations- as an in reach nurse to be able to respond within 1-2 working days for urgent contacts which will lead to better outcomes for both services and service users

Reducing impact on Older Adult Community Mental Health clinicians- as an reach nurse being able to support junior staff Nurses with community care home referrals

The main functions of the care home in reach nurse is to support patients within the care home setting to try and prevent placement break and reduction in admission to both general and mental health hospitals 

In addition the care home in reach nurse will lead on completing Diadems assessments using the Diadem tool to reduce waiting times for patients within care homes who do not have a diagnosis of dementia and support with an increase diagnostic rates  within care homes

The care home in reach nurse will provide a quicker response to patients to improve the service response time in both routine and urgent referrals .

The care home in reach nurse will promote collaborative working with care homes across Coventry and Warwickshire, supporting them to meet the needs of Older Adult patients through enhanced communication, modelling of good practice, skills and knowledge.

The care home in reach nurse will deliver strategies and support to enable residents to remain in the care home through crisis avoidance and stabilisation. Reducing placement breakdown and improved patient flow of dementia/ Older Adult patients to care homes. The role reduces the impact on Older Adult Community Mental Health clinicians by providing support with formulation and care planning.

The successful applicant will work across Coventry and Warwickshire to support patients within a care home setting with a focus on placement breakdown, admission avoidance and increase memory assessment using the Diadem assessment tool. 

As a clinician within the team, we will ensure that you have clinical and professional support from the relevant professionals to support the maintenance of your professional registration. This support will be alongside the supervision and management. 

Communicate with both internal and external stakeholders, such as service users, carers, other teams, third sector and voluntary organisations on matters which may be complex and/or contentious.

Communicate effectively across a wide variety of settings, using tact and diplomacy in response to situations that may be complex and sensitive and may have resulted from conflict.

Providing advice and use their initiative and follow organisational procedures in routine and crisis situations

Apply communication skills which take account of other communication needs and differences, in order to plan, deliver and evaluate care.

Apply enhanced communication skills both oral and written to communicate information within records and reports within their service area.  This may also include the need to respond to and offer and reassurance to questions and concerns from a variety of groups, such as service users, carers and other staff/teams.

Comply with and implement local and Trust policies and procedures regarding appropriate communication.

Be required to lead client reviews within the multi-disciplinary team and act as an advocate for service users

Analyse and act on multiple and complex information assuring adherence to local and national guidance, legislation and policy

Act within the limits of their competency and authority in line with their professional code of conduct.

Will manage and organise competing and conflicting priorities.

Review, evaluate and adjust activities in response to changing demands

Have the ability to use I.T. systems sensibly and purposefully to meet the needs of the role and will be required to utilise systems safely and securely in line with organisational guidelines.
