Summary

The post holder will have responsibility for participating in the delivery of patient care under the supervision of the Community Team. * Work under supervision following written care plans and care pathways devised by the Registered Nurse. * Follow prescribed care plans and treatment protocols. * Follow HPCT Policies and Procedures. * Has knowledge and skills to carry out prescribed tasks and will report to Registered Nurse any variances. * Delegated responsibility for Patient Support Teams administrative tasks. * Provide general administration support to the team. This role will demonstrate excellent communication skills, using them to engage, persuade or negotiate with clients with complex, at times challenging needs to ensure they are provided with the most appropriate care and support. The Team consists of Occupational Therapists, Physiotherapists, Nurses, Doctors, Consultants, Social Workers, and Healthcare Assistants who work together in an integrated way to provide co-ordinated, clinical support to promote independence and to improve quality of life.
