Summary

An exciting opportunity has arisen for a highly motivated and enthusiastic individual to join the Hull and East Riding Intermediate Care Team as a Bank Generic Therapy Associate Practitioner. You will work as part of a Multidisciplinary Team within a community rehabilitation unit, delivering safe, high-quality therapy interventions and rehabilitation programmes under the guidance of a therapist.

This role helps people in the community improve their health, wellbeing and independence, making a real difference to their daily lives.

As an Bank Associate Practitioner, you will work as part of a supportive multi-disciplinary team, delivering high-quality, evidence-based care and rehabilitation interventions. You will manage a defined caseload, support people in their homes and community settings, and help them achieve their health and wellbeing goals. The role also involves working closely with registered clinicians, support workers, carers and partner organisations to ensure patients receive timely and effective care.

What you’ll do:

• Deliver rehabilitation and wellbeing interventions that support people to maintain or improve independence

• Support assessments, care planning and reviews under the guidance of registered clinicians

• Monitor patient progress and escalate concerns when needed to ensure safe care

• Work closely with patients, carers and partner agencies to coordinate support

• Supervise and support less experienced colleagues and support workers

What we’re looking for:

• A relevant Level 4 qualification, Foundation Degree in Health and Social Care, or equivalent rehabilitation experience

• Experience working in a clinical, health, care or rehabilitation setting

• Strong communication and organisation skills, with a caring and person-centred approach

• The ability to work independently while knowing when to seek support from registered professionals

• A commitment to teamwork, safe practice and helping people achieve their goals
