Summary

Working with the Stroke Therapy Team, you’ll support people recovering from stroke so that they can regain independence, confidence and quality of life. You'll play an important role in delivering rehabilitation programmes and helping patients achieve their personal goals.

As part of a friendly multidisciplinary team, you’ll work alongside Occupational Therapists, Physiotherapists, Speech and Language Therapists, nurses and other healthcare professionals. This role combines hands-on patient care with the opportunity to develop specialist rehabilitation skills while making a meaningful difference to patients and their families

What you’ll do:

• Deliver rehabilitation programmes to support mobility, daily living, communication and swallowing needs

• Monitor patient progress and report concerns or changes to supervising clinicians

• Support patients, families and carers with information, education and encouragement

• Maintain accurate patient records and contribute to multidisciplinary working

• Promote independence, wellbeing and self-management throughout recovery

What we’re looking for:

• A relevant NVQ Level 4, Foundation Degree in Health and Social Care, or equivalent rehabilitation experience

• Experience working in a clinical care setting and as part of a multidisciplinary team

• Good communication, organisational and interpersonal skills

• The ability to work independently within agreed competencies and know when to seek support

• A commitment to delivering compassionate, person-centred care
