Summary

We are looking for an enthusiastic and motivated Assistant Practitioner to join our Stroke Rehabilitation Team. The successful candidate will work closely with therapists and the wider multidisciplinary team to support patients to maximise their independence and achieve their rehabilitation goals. Key duties include: Delivering rehabilitation programmes for patients following stroke and other neurological conditions. Managing a delegated caseload and monitoring patient progress within agreed competencies. Supporting patients and families with rehabilitation, discharge planning and self-management strategies. Working collaboratively with therapists, nursing colleagues and community services to ensure seamless patient care. Maintaining accurate clinical records and contributing to high standards of care and governance. Supporting service development, quality improvement initiatives and the delivery of a seven-day rehabilitation service. This is an excellent opportunity for an experienced rehabilitation support worker or assistant practitioner who is passionate about delivering high-quality, person-centred care and helping patients achieve the best possible outcomes.
