Summary
Main Duties of the Job Service Improvement & Transformation Work with practices to understand operational challenges and identify practical solutions, including sharing best practice across the PCN. Lead the planning, prioritisation and delivery of changes that improve patient access, staff experience and service efficiency. Delivery of KPIs, Develop and maintain a PCNwide transformation plan, managing interdependencies, risks and resource requirements to deliver KPIs. Support staff to access relevant training and development to enable successful delivery of agreed improvements. Use recognised project and improvement methodologies to track progress and evaluate impact. Key Performance Indicators (KPIs) KPIs will be agreed locally and may include: Increased uptake of NHS App and online consultation tools. Improved EMIS data quality and coding consistency. Measurable improvements in patient access and digital experience. Demonstrable reduction in unwarranted variation across practices. Delivery of agreed PCN-led programme transformation milestones. Delivery of Population health management Programme across Integrate Neighbourhood teams. Information & Data Management Take a datadriven approach to understanding challenges, identifying solutions and measuring improvement. Work with practices, INT and ICB to standardise data sets and metrics used for monitoring change. Bring together data from multiple sources to generate actionable insights for practices. Present data clearly to clinical and nonclinical audiences to support decisionmaking. Highlight unwarranted variation and support practices to understand data limitations. Encourage a culture of using data to evaluate operational processes and track trends over time. Secure analytical support where required and strengthen PCN datasharing mechanisms. Communication & Engagement Build strong working relationships with practices, INT, ICB and wider stakeholders. Support consistent communication and engagement across the PCN to ensure buyin to change. Ensure changes align with clinical priorities and have ownership from clinical leaders. Support practices to communicate new processes effectively to patients. Identify and share best practice from other PCNs and systems. Participate in relevant working groups, project boards and forums. Policy & Service Development Maintain awareness of national and regional policy developments and support practices to interpret and apply them locally. Act as Information Governance Lead for Sphere PCN. Work with the ICB to support development and delivery of primary care and digital transformation strategies. Support practices to adapt policies and initiatives to local needs and priorities. Equality & Diversity Promote equality, diversity and the rights of patients, carers and colleagues.
