Summary
You will join a highly experienced team of specialist Neurological Clinicians, with excellent opportunities for collaborative and joint working. The post holder will be responsible for managing a physiotherapy caseload within the HCT Community Neuro Service as part of our Integrated Community Stroke and Neuro Service (ICSNS), providing specialist assessment and treatment for patients with a variety of neurological conditions and needs. The role involves working across East and North Hertfordshire in a variety of community settings. As a team, we work closely with health and social care partners, carers, and families to deliver high-quality, person-centred neurological rehabilitation.

We are looking for an enthusiastic and motivated Physiotherapist to join our Integrated Community Stroke and Neuro Service. You will manage a specialist caseload of patients with neurological conditions, providing assessment, treatment and rehabilitation within patients' homes and a variety of community settings across East and North Hertfordshire.
Working as part of a highly experienced multidisciplinary Community Neuro Team, you will deliver person-centred care for individuals with a wide range of neurological conditions and rehabilitation needs. You will work closely with health and social care colleagues, carers and families to support patients in achieving their goals and maximising their independence.
The role includes participation in multidisciplinary team meetings, supervision and support of junior staff and students, and providing specialist advice and guidance to colleagues, patients and carers. There may also be opportunities to gain experience across different areas of the service, including specialist neuro rotations.
We currently provide a six-day service and are working towards a seven-day service model. The successful candidate will be expected to participate in the weekend rota, currently working approximately one Saturday in every six.

Main duties of the job
We are looking for an enthusiastic and motivated Neurological Physiotherapist to join our Integrated Community Stroke and Neuro Service. You will work as part of a highly experienced multidisciplinary team, delivering specialist assessment, treatment and rehabilitation for adults with stroke and a wide range of neurological conditions.
Managing your own caseload, you will provide person-centred rehabilitation across a variety of settings, including our Neurological Rehabilitation Unit, outpatient clinics, and patients' homes across East and North Hertfordshire. You will support patients with complex neurological rehabilitation needs to maximise their independence, wellbeing and quality of life.
Working closely with patients, carers, families and health and social care colleagues, you will contribute to coordinated care planning and provide specialist advice and education to support rehabilitation outcomes. You will also play a key role in multidisciplinary team working, supervision of students and support staff, and service development activities including audit and quality improvement.
This role offers opportunities to broaden your experience across different areas of the Hertfordshire Community Neuro Service, including potential participation in specialist neuro rotations. The post holder will also participate in the service weekend rota, which currently involves approximately one Saturday in every six weeks.
