Summary
Job description Job responsibilities The QOF care coordinator will support the delivery of high-quality patient care by coordinating Quality and Outcomes Framework QOF activities, enhanced services, patient recall programmes, and data quality initiatives across the practice. The post holder will work closely with clinicians and administrative staff to ensure that contractual targets are met, patient records are accurately maintained, and performance is monitored effectively. QOF and Performance Management Monitor and support the achievement of QOF targets and other contractual requirements. Run regular clinical searches and reports to identify patients requiring review, monitoring, or intervention. Maintain and manage patient recall systems for chronic disease management, screening programmes, immunisations, and health checks. Support clinicians in identifying care gaps and opportunities to improve patient outcomes. Produce performance reports and information for meetings.
