Summary

1. Prescription Administration a)Process routine repeat prescription requests received through thePractice's approved systems. b) Accurately check prescriptionrequests against the patient's electronic medical record and current repeatmedication list. c) Process requests in accordancewith Practice prescribing procedures and agreed turnaround times. d) Ensure prescription requests are accurately recorded and appropriately actioned. e) Identify requests that falloutside standard repeat prescribing processes and refer them to the appropriateclinician. f) Deal with prescription requestsfrom patients, carers, pharmacies, care homes and other authorised healthcareprofessionals. g) Monitor outstanding prescriptionrequests and undertake appropriate follow-up. h) Ensure prescription-relateddocumentation is accurately recorded and maintained and assist with theadministration of prescription queries and medication-related correspondence. j)Support agreedmedication review and recall processes from an administrative perspective. k)Assist with prescription-related audits and searches. l)Support the Practice inidentifying outstanding medication-related administrative actions. m)Assist with medicines management projects and quality improvementinitiatives. n)Maintain awareness of Practice procedures relating to high-riskmedicines, controlled drugs and other medicines requiring additionalsafeguards, escalating requests appropriately. 2. Patient and Health care Professionals Communication a) Provide a professional, courteous and helpful service b)Respond to telephone, face-to-face and electronic prescription enquiries. c) Explain Practice procedures and prescription turnaround times clearly and sensitively d)Deal appropriately with patients who may be distressed, anxious or frustrated. e)Always maintain patient dignity and confidentiality f)Recognise when a patient's query is clinical in nature and ensure it is directed to an appropriate healthcare professional g)Provide appropriate signposting where the query falls outside the responsibilities of the Prescription Clerk h)Communicate effectively with community pharmacies regarding prescription-related administrative queries. i) Liaise with care homes and other authorised organisations regarding prescription requests where appropriate. Refer clinical queries to the GP, Clinical Pharmacist, Pharmacy Technician or other appropriate healthcare professional. Assist in resolving discrepancies or administrative issues relating to prescriptions. l) Ensure information received from external organisations is passed to the appropriate member of the Practice team. 3. Information Governance and Confidentiality a) Maintain strict confidentiality in accordance with Practice policies and relevant legislation. b) Comply with UK GDPR and the Data Protection Act 2018. 4. Record Keeping and IT a) Maintain accurate and timely electronic records. b) Use the Practice clinical system and relevant administrative systems effectively. c) Ensure correspondence and documentation are processed in accordance with Practice procedures. 5. Communication and Team Working a) Work collaboratively with GPs, Clinical Pharmacists, Pharmacy Technicians, Nurses, Healthcare Assistants, Receptionists and Administrative staff. b) Participate in relevant Practice meetings and team briefings. 6. Safeguarding a) Understand responsibilities in relation to safeguarding children and adults. b) Complete safeguarding training appropriate to the role. c) Recognise and appropriately escalate safeguarding concerns. d) Follow Practice safeguarding policies and procedures. 7. Equality, Diversity and Inclusion a) Treat all patients, carers, visitors and colleagues with dignity, fairness and respect. b) Promote equality and diversity in all aspects of work. 8. Health, Safety and Infection Prevention a) Comply with Practice health and safety policies.
