Summary

Transition Co-ordinator (SEND)

Are you passionate about helping young people with SEND achieve their goals and thrive in adulthood?

Join our dedicated SEND team and play a key role in supporting learners, particularly those with EHCPs, as they transition from school to college and beyond. This is a full-time role (37 hours per week) working across our vibrant Amersham and Wycombe campuses.

What You'll Do

Support smooth and successful transitions for learners with SEND.

Build strong relationships with schools, families, local authorities, and external partners.

Coordinate EHCP consultations, reviews, and transition planning.

Organise engaging transition events for prospective students and families.

Promote progression to employment, independent living, and community participation.

Empower learners to develop confidence, independence, and work-ready skills.

About You

You will have:

Experience supporting young people with SEND.

Knowledge of EHCP processes and the SEND Code of Practice.

Excellent communication and organisational skills.

A commitment to improving outcomes and opportunities for learners.
