Summary

Transition Co-ordinator (SEND)

Are you passionate about helping young people with SEND achieve their goals and thrive in adulthood?

Join our dedicated SEND team and play a key role in supporting learners, particularly those with EHCPs, as they transition from school to college and beyond. This is a full-time role (37 hours per week) working across our vibrant Amersham and Wycombe campuses.

What You'll Do

  • Support smooth and successful transitions for learners with SEND.
  • Build strong relationships with schools, families, local authorities, and external partners.
  • Coordinate EHCP consultations, reviews, and transition planning.
  • Organise engaging transition events for prospective students and families.
  • Promote progression to employment, independent living, and community participation.
  • Empower learners to develop confidence, independence, and work-ready skills.

About You

You will have:

  • Experience supporting young people with SEND.
  • Knowledge of EHCP processes and the SEND Code of Practice.
  • Excellent communication and organisational skills.
  • A commitment to improving outcomes and opportunities for learners.
