About the Role

We are looking for an experienced and motivated Care Co-ordinator to join our growing team, supporting services across Oxford, Banbury and High Wycombe.

This is a rewarding, field-based role where you will play a key part in ensuring high-quality, person-centred support is delivered to the adults and children who use our services. You will work closely with the people we support, their families, carers and wider professionals, helping to co-ordinate safe, effective and responsive care.

Our head office is based in Nottingham, but this role will be field-based across the local service area. There is no weekend, out-of-hours or on-call requirement.

Key Responsibilities

As Care Co-ordinator, your responsibilities will include:

• Building strong, professional relationships with customers, families, carers and external professionals

• Supporting the co-ordination and planning of care and support packages

• Ensuring services are delivered in line with individual needs, care plans and regulatory requirements

• Communicating effectively with carers and support staff to help ensure safe and consistent service delivery

• Supporting quality monitoring, reviews and service improvements

• Maintaining accurate records and using internal systems confidently and effectively

• Working collaboratively with colleagues to support the growth and development of the service

• Representing the company professionally while working in the field

About You

We are looking for someone who is organised, confident and people-focused, with strong communication skills and a genuine commitment to high-quality care.

You will need:

• 2–3 years’ experience working in a CQC-regulated service

• Experience working in adult or children’s services

• Excellent interpersonal skills, with the ability to build rapport with customers, families and carers

• Good IT skills and confidence using digital systems

• Strong organisational skills and attention to detail

• A proactive and flexible approach to work

• A full UK driving licence and access to your own car

Experience supporting people with learning disabilities and autism would be ideal, but it is not essential. Additional training and personal development will be provided.

Hours and Flexibility

This is a full-time role, with core working hours of Monday to Friday, 9am–5pm.

We are also happy to consider part-time or reduced hours for the right candidate.

There is no weekend working, out-of-hours or on-call element to this role.
