Here is a summary: - Engage individuals admitted to hospital due to alcohol-related issues and facilitate access to treatment and recovery services.- Deliver brief interventions, harm reduction advice and motivational support to encourage behaviour change.- Complete initial screening assessments and identify risks, safeguarding concerns and support needs.- Support service users following discharge through telephone, digital and face-to-face follow-up interventions.- Work collaboratively with hospital teams, community services and external stakeholders to improve pathways into treatment and recovery.- Support service users to access wider community resources, strengthening recovery capital and reducing social isolation.- Promote visible recovery and encourage engagement with peer support and recovery communities.- Maintain accurate and timely records, reports and outcome data in line with organisational requirements.- Contribute to partnership meetings, service development initiatives and training opportunities with hospital and community partners.
