Summary

The post holder will work as part of the DART clinical team, providing safe, effective and compassionate care to patients with drug and alcohol related needs. Responsibilities will include undertaking substance misuse assessments, supporting patients through treatment pathways, administering prescribed medication and treatments, monitoring patients during treatment, and identifying and responding to changes in their clinical presentation. You will contribute to nurse-led substance misuse clinics, provide health education and harm reduction advice, support recovery-focused care, and make appropriate referrals to other healthcare professionals and services where required. The role involves working closely with colleagues across DART, Primary Care, Mental Health and wider partner services to ensure care is coordinated and responsive to individual patient needs. The successful candidate will be expected to maintain high standards of clinical documentation, work in line with relevant policies and clinical guidance, contribute to multidisciplinary working, and support the delivery and continued development of safe, high-quality substance misuse services at HMP Humber.
