Summary
Role Duties Clinical Practice Independently assess, examine, diagnose, and manage patients presenting with acute illness and acute exacerbations of chronic disease. Provide expert management of conditions including: COPD and asthma exacerbations Heart failure decompensation Diabetes complications Frailty and complex multimorbidity Acute infections in patients with chronic disease Chronic pain flare ups Hypertension and cardiovascular instability Request, interpret, and act upon diagnostic investigations. Prescribe medications independently within scope of practice. Develop and implement evidence based management plans. Identify deteriorating patients and escalate appropriately. Deliver same day urgent care clinics and home visit assessments where required. Please see attached job description/person specification for full details.
